Carnival Order Form:

 The Carnival Order Form will be coming home in your child's round-trip folder the week of April 22nd. If you do not receive it please check with your child's teacher.

This form allows you to pre-order wristbands and raffle tickets before Carnival night. Each raffle ticket is $2. Each wristband is $10 and includes access to all games and activities plus $5 worth of food. Wristbands are for children only, adults do not need to purchase wristbands. Additional food can be purchased on Carnival night with cash or check. We highly encourage you to pre-order as it makes logistics much simpler for our Carnival planning team! Don’t worry, we know that life can get hectic, so we will also have these available for purchase in-person on the night of Carnival.

Pre-ordering your raffle tickets and wristbands is easy – just follow these steps:

• On your Carnival Order form, you will see a list of numbers. Each number corresponds to one of our raffle items. Refer to our eBook (link above) to see the full list of the raffle items, each with their own corresponding number. 
• After reviewing the list of raffle items and deciding which items your family would like to purchase tickets for, just write the number of tickets (chances to win) you would like for each item next to the corresponding item number.
  • For example, if you wish to purchase 10 tickets for Lunch with Dr. Dahlquist (which will give you 10 chances to win this item), write “10” next to the number 1, which is the corresponding item number for this item in the Carnival eBook.
• After you have finished choosing the number of tickets you wish to purchase for each raffle item, simply add up the total number of raffle tickets at the bottom of the form and multiply by $2 (the cost per raffle ticket). 
• For your wristbands, write down how many you would like to purchase (one per child, you may also purchase wristbands for siblings who do not attend Creek Valley). Adults do not need wristbands.
• To complete your form, add the wristband and raffle ticket totals up and send the form (original and copy) back to school with cash or check (made out to Creek Valley PTO) in the amount of the Grand Total you calculated at the bottom of the form.
• That’s it! We will take care of the rest.
  • We will keep track of your claim numbers on the back end and will place the raffle tickets you purchase into their corresponding Item Ticket Box(es). The Ticket Boxes will be out the night of Carnival in case you want to purchase more tickets and add to your chances of winning!
  • We will deliver your wristbands and your raffle claim ticket number(s) stapled to a copy of your order form on the night of Carnival, look for them in your oldest child’s locker.

Carnival Bags:

When you arrive at Carnival go to your oldest child's locker to find our Carnival Bags, which will contain:

- Copy of your Carnival Order Form with raffle claim ticket(s)

- Wristbands (one per child)

- Point Card on lanyards (1 lanyard per child with one point card each)
